Create and manage multiple contact forms in Magento 2.
By taking advantage of the new robust platform of Magento 2, this extension allows you to create unlimited contact forms in your store. It comes with built-in interface for user-defined form fields which allows for unique forms as per your requirements. Unlike other similar solutions, here you can define form fields yourself, choose fields type, change order of appearance, define field validation patterns and more.
All form submissions are saved and accessible from Magento 2 administration panel. Created forms can be displayed by widgets and/or directly in your templates. Forms can be displayed in both CMS and Product detail pages.

EAV Attribute based
We have developed the extension to work on top of native Magento 2 EAV attributes system. This allows you to create forms with virtually ANY type of attributes supported.
Being EAV based you can create any type of field, assign desired validation and behaviour. This also allows any third party modules that add custom field types to hook up to form fields.
Attribute Options Supported
You can set the same attribute options that can be assigned to any native Magento 2 attribute. This includes field (attribute) type, validation, behaviour and much more.
Unlimited forms
You can create unlimited number of forms with dynamic fields. Each form can have multiple recipients, custom subject, custom set of fields.
Form submission is AJAX based and it supports reCapcha out of the box. Form submissions are also recorded and displayed in the product edit form.

User Guide & Documentation
Installation from Command Line
This extension can be installed in a few minutes, by going through the following steps:
- Download/purchase the extension
- Unzip the file in a temporary directory
- Copy directory contents and paste it in your Magento installation root directory. This extension will not replace existing code or file structure
- Enter the following at the command line:
php bin/magento setup:upgrade
php bin/magento setup:static-content:deploy (only if site is in production mode)
Installation and updating with Composer
You can install this extension via Composer only after purchasing a Composer support package
- Run the following Composer commands
composer require anowave/package
composer require anowave/contact
bin/magento setup:upgrade
bin/magento setup:static-content:deploy (only if site is in production mode)
Upgrading database
If you need to upgrade the database, run the following command line:
php -f bin/magento setup:db-schema:upgrade
Configuration
To enable extension go to Stores -> Configuration -> Anowave Extensions -> Contact Forms
Follow the instructions on the screen to configure your extension.
Creating Form Fields
To create form fields go to Stores -> Attributes -> Form and create your form fields. (See below)
Creating Forms
Once you have created your fields it's very easy to create the form itself. To create new form follow the steps below
- 1. Go to Content -> Contact forms and click "Add contact form"
- 2. Fill in the form options (see below)
Adding contact form to standard CMS page in Magento 2
Contact forms can be added to any standard CMS by using widgets. To add a form click "Insert widget", select "Contact from" and then select the desired form to add. Once page is saved, form will appear in frontend.
Adding enquiry form to products in Magento 2
Any custom form can be used also as a product enquiry form. You can assign any form to product by simply editing the product itself. The extension adds a new tab in product edit screen where you can select which form should be used as product enquiry form. These forms will appear in the tabs on product detail page. (See below)
